You ever notice that investigations for aviation incidents are always like "These three components failed simultaneously, at which point the pilot mistakenly followed the checklist for a slightly different failure mode, compounded by a miscommunication with air traffic control due to the proximity of another flight with the same call sign", and investigations for maritime incidents are always like "They forgot not to crash"?
