Woohoo! My first open source contribution is out in the wild.
If you are using @IceCubesApp you can now go to Settings/Content Settings and enable the Require Alt Text to Post feature.

If you attempt to post media without alt text you get this lovely error!

There is only an “OK” button to dismiss the modal to back to the previous step.
